Hubbard Fun Fest & Orientation - All Grade Levels

Dear Parents and Scholars, 

On behalf of everyone at Hubbard High School and with great excitement, we welcome our scholars to the return of in-person learning on Monday, August 30.  We look forward to establishing and maintaining a strong relationship  ensuring that all of our scholars have a robust learning experience. 

The activity fee is $150.00 per student. This covers the cost of class fees, technology fee, lock, one ID, one Hubbard lanyard, free admission to all HOME athletic contests, and the yearbook. Credit/debit card payments can be made on our website through e-PAY. Cash and money order payments made payable to Hubbard High School will be accepted during the fun fest. Unfortunately, personal checks are NOT accepted. PE uniforms and Ventra cards will begin being sold on the first day of school, August 30, 2021. Please note- There will be a one time per school year $50.00 athletic fee that every student athlete will have to pay, regardless of the number of sport teams they will be participating in during the school year.

 

We ask that you follow the most updated health and safety guidelines that have been established by the Centers for Disease Control and Prevention (CDC), City of Chicago, and CPS to ensure the health and safety of those in and around the school building. 

We will use our rear parking lot located on 62nd street and Springfield to complete our orientation and registration activities. Please note, that  you will not be able to park in the parking lot, but you may park around the building

ALL scholars must turn in the following forms: Income Verification (English), Income Verification (Spanish) the Media Consent (English), Media Consent (Spanish) and the Student Acceptable Use forms. We are asking that you also turn in an Emergency Contact form if you have updates to phone numbers or emergency contact names. The forms are available on our website. Forms will also be available for pick up August 9-10 from 9am until 12pm at Door 8.

Completed forms will be collected during the fest. For each form that is returned, scholars will be automatically entered into our raffles. 

 Freshman/ Upperclassmen Transfer Student Registration Information: If you HAVE NOT registered your student, you may do so during the Scholar Fun Fest. Please bring the following documentation with you: 

  1. Copy of the student’s Birth Certificate
  2. Copy of the parents’/legal guardians’ State ID or Driver’s License, must be up to date with current address
 
  1. Proof of legal guardianship is required for any adult registering the student, other than the parents listed on the birth certificate.
  2. Proof of residency - (Two of the following) Drivers license/State ID, utility bill with current address in name of the parent/legal guardian, MediCare/Medicaid card, W2 forms, Voter’s Registration Card or court documents.  Rental lease contracts and/or payments are not accepted as proof of residency.
  3. Student’s Physical Examination and Immunization Record
  • Physical exam should be no more than a year old
  • Must meet all immunization requirements
  • Physical and immunizations must be submitted not later than October 15.
